Community Nursing Care – NDIS Support at Merci Care

Our Services

At Merci Care, we provide high-quality Community Nursing Care under the NDIS to support participants with complex health needs in the comfort of their own home or community setting. Who is Community Nursing Care for?

Community Nursing Care is available to NDIS participants who require clinical or personal nursing support due to:

  • Chronic health conditions
     
  • Medication management needs
     
  • Wound care or pressure injury management
     
  • Diabetes support (e.g., insulin administration)
     
  • Stoma, catheter, or continence care
     
  • PEG feeding or enteral nutrition

These services fall under Capacity Building Supports – Improved Daily Living or Core Supports, depending on your NDIS plan and needs.

Our Process – What to Expect


1. Initial Contact and Enquiry

Reach out to us via phone, email, or our online contact form. Our friendly intake team will answer your questions and guide you through the next steps.

2. Needs Assessment

We’ll arrange a no-obligation consultation (in person or over the phone) to understand your clinical needs, current health status, and the level of support required. We also review your NDIS plan to check for eligibility and funding availability.

3. Care Plan Development

Our Registered Nurses work with you (and your family or support coordinator, if needed) to create a personalised nursing care plan. This plan outlines the type of services you’ll receive, how often, and how they support your goals.

4. Service Agreement and Booking

Once you're happy with the plan, we’ll finalise a simple Service Agreement and schedule your nursing visits at times that suit you.

5. Ongoing Support and Reviews

Our nurses will provide care according to your plan—compassionately, professionally, and with respect. We regularly monitor your progress and update your care plan as needed to ensure it continues to meet your goals and clinical needs.
